How do I sign up for the Patient Portal?
Signing up for the portal is easy; simply provide your email address during your next visit to any Aylo Health office, and you will receive a username, temporary password, and link to the portal. From there, log in to create a new password.
How do I schedule an appointment?
To schedule a visit at one of our offices, you can book an appointment online or call (770) 268-4011 to speak with a Patient Service Rep.
How do I access my medical records?
To view most of your medical records, simply log in to your patient portal above. If you can’t find what you’re looking for, please call our office at 770-268-4011.
How do I find accepted insurance information?
Aylo Health accepts most major insurance plans, including; Aetna, Blue Cross Blue Shield, Cigna, Humana, Medicare, Tri-Care, and United Healthcare. Please note that benefits and coverage are specific to patient plans. We recommend contacting your insurance company to verify that our providers are in your network, ensuring that your office visits will be covered.
