What is Remote Patient Monitoring (RPM)?
Through RPM, you’ll be supported by a dedicated care advisor and your Aylo Health provider. This team will help you monitor your health, set personal goals, and stay ahead of any potential issues—so we can keep you feeling your best and help you avoid unnecessary ER visits or hospital stays.
Remote Patient Monitoring Programs Offered
Glucose Monitoring
We’ll provide you with a glucometer that automatically sends your readings to our secure system every day. That way, your care team can keep a close eye on your health in between your follow-up visits.
Here’s what you can expect if you decide to join:
· Daily glucose readings will be sent in by you using the device we provide so that your care team can see an accurate trend in readings.
· Ongoing support from your Care Advisor, including regular check-ins and personalized care based on your results.
In just four months, patients who have enrolled in the glucose remote monitoring program have successfully reduced their A1C levels by 2.5 points.
High Blood Pressure Monitoring
We’ll provide you with a blood pressure monitor that automatically sends your readings to our secure system every day. That way, your care team can keep a close eye on your health in between your follow-up visits.
Here’s what you can expect if you decide to join:
· Daily blood pressure readings using the device we provide—your readings will automatically upload to your care team.
· Ongoing support from your Care Advisor, including regular check-ins and personalized care based on your results.
In just three months, their patients saw an average blood pressure improvement of 11 points systolic and 7 points diastolic—a meaningful step toward better long-term health.
Weight Monitoring
We’ll provide you with a weight scale that automatically sends your readings to our secure system every day. That way, your care team can keep a close eye on your health in between your follow-up visits.

· Daily readings will be sent in by you using the device we provide so that your care team can see an accurate trend in readings.
· Ongoing support from your Care Advisor, including regular check-ins and personalized care based on your results.
Patients with congestive heart failure who have enrolled in RPM have achieved consistent weight stability, with a marked reduction in fluid retention. These patients maintained an average weight fluctuation of less than 2 pounds per week over a three-month period.

Remote Patient Monitoring FAQs
Is the RPM program complicated?
No. Our program uses cellular function to send the vital signs so you just take a reading as normal with our easy-to-use device from any location within the United States.
Is RPM secure?
Yes. Your health information is secure. We will never share this information without your permission.
What are the program requirements?
Daily readings allow a better view of your health and assist in providing the best possible care. Our team can even set up reading reminders.
A monthly call will be completed to discuss your readings, concerns and overall health and wellness.
Is RPM covered by my insurance?
Medicare Part B covers RPM. With most supplemental coverage, there is no cost to you. Prior to start of RPM program, we will determine what your RPM coverage is and if there will be a copay.
Will I need to have access to Wi-Fi or download an app to participate?
You will not need access to Wi-Fi. Your readings will be transmitted through cellular functions, so Wi-Fi is not required. No apps are required either. All you need to do is take a reading, and your care team will handle the rest.
Can the device be shared with someone else?
No. The device and/or supplies that you receive are only to be used by you. We are tracking trends in your readings, so sharing with others would provide inaccurate information.
