POSITION PROFILE:
We seek a compassionate, detail oriented, and outcomes driven Clinical Quality Outreach Coordinator to join our Quality team. The Coordinator primarily completes Transition of Care Management & Medication Adherence outreach. The Coordinator also assists the team with other Clinical Quality activities. The objectives include:
- Timely outreach to recently discharged patients
- Intervention to enhance patient medication compliance
- Closing gaps in patient care
- Ensuring documentation accurately reflects the patients’ condition(s)
KEY RESPONSIBILITIES:
- Reviewing, prioritizing, and actioning on daily discharge reporting to include calls to patients & caregivers
- Reviewing, prioritizing, and actioning on payor medication adherence reporting to include calls to patients, patient caregivers, and pharmacies
- Performing chart reviews for documentation of quality care
- Reviewing reports, data, and medical records to identify opportunities to improve care quality and identify trends related to care quality
- Communicating with clinical team members to achieve program objectives
- Participating in meetings and calls with internal constituents and payors as necessary
- Assisting with other duties as assigned
REQUIRED QUALIFICATIONS:
- High School graduate or equivalent education required.
- Working knowledge of the patient record in the EMR (preferably eCW).
- Medical knowledge associated with experience such as MA, LPN, biller/coder as relates to clinical quality including medical terminology, diagnoses, and medications.
- Excellent verbal and written communication skills.
- Ability to effectively utilize technology to perform clinical quality tasks.
- Prefer previous experience with clinical quality initiatives.
- Knowledge of HEDIS and Medicare.
- Demonstrated ability to organize, coordinate, prioritize, and facilitate multiple tasks.
- Proficient computer skills including Excel, Word, Microsoft Office, Outlook and others
- Professional demeanor and appearance.
- Ability to contribute to a positive, supportive work environment and collaborate with a team.
