POSITION PROFILE:

We seek a compassionate, detail oriented, and outcomes driven Clinical Quality Outreach Coordinator to join our Quality team. The Coordinator primarily completes Transition of Care Management & Medication Adherence outreach. The Coordinator also assists the team with other Clinical Quality activities. The objectives include:

  • Timely outreach to recently discharged patients
  • Intervention to enhance patient medication compliance
  • Closing gaps in patient care
  • Ensuring documentation accurately reflects the patients’ condition(s)

KEY RESPONSIBILITIES:

  • Reviewing, prioritizing, and actioning on daily discharge reporting to include calls to patients & caregivers
  • Reviewing, prioritizing, and actioning on payor medication adherence reporting to include calls to patients, patient caregivers, and pharmacies
  • Performing chart reviews for documentation of quality care
  • Reviewing reports, data, and medical records to identify opportunities to improve care quality and identify trends related to care quality
  • Communicating with clinical team members to achieve program objectives
  • Participating in meetings and calls with internal constituents and payors as necessary
  • Assisting with other duties as assigned

REQUIRED QUALIFICATIONS:

  • High School graduate or equivalent education required.
  • Working knowledge of the patient record in the EMR (preferably eCW).
  • Medical knowledge associated with experience such as MA, LPN, biller/coder as relates to clinical quality including medical terminology, diagnoses, and medications.
  • Excellent verbal and written communication skills.
  • Ability to effectively utilize technology to perform clinical quality tasks.
  • Prefer previous experience with clinical quality initiatives.
  • Knowledge of HEDIS and Medicare.
  • Demonstrated ability to organize, coordinate, prioritize, and facilitate multiple tasks.
  • Proficient computer skills including Excel, Word, Microsoft Office, Outlook and others
  • Professional demeanor and appearance.
  • Ability to contribute to a positive, supportive work environment and collaborate with a team.
